I did this:
Post Type Title Template:
%%title%% %%sep%% %%pt_plural%% %%sep%% %%ct_location%% %%sep%% %%sitename%%

I expected the plugin to do this:
SEO Title to display:
sjoelen - games - netherlands - europe - sitename

Instead it did this:
SEO Title to display:
sjoelen - games - netherlands - sitename

%%ct_location%% is a hierarchical taxonomy including continents at level 1 and countries at level 2 eg: /europe/netherlands/

Is it possible to include more than level of the taxonomy ?
